The Power of Partnerships: How the Free Clinic of Powhatan Connects Patients to Specialty Care
A primary care appointment can be the beginning of something much bigger. Sometimes, a patient's needs can be addressed right here at the Free Clinic of Powhatan. Other times, our medical team recognizes that a patient needs a level of specialized care that goes beyond what we provide in our clinic. That's where partnerships come in.
Through organizations like Access Now, volunteer specialists, and generous medical practices throughout our region, the Free Clinic of Powhatan helps connect patients to the specialty care they need—often at no cost to them. For our patients, that connection can be life-changing.
It Starts With a Primary Care Visit
One of the most important things we provide our patients is a medical home. When a patient has an ongoing relationship with a primary care provider, their healthcare needs can be identified and addressed over time. Our doctors and nurse practitioners may recognize that a patient's symptoms require additional testing, a specialist's expertise, or a procedure that cannot be provided at the clinic.
When that happens, the referral process begins. Our provider identifies the need and connects with our case manager, who works to help the patient navigate the next step. For someone who has never had consistent access to healthcare, knowing where to go, how to get there, what to expect, and what happens next can be overwhelming.
A Referral Is More Than a Piece of Paper
At the Free Clinic, a referral doesn't simply mean handing a patient a phone number and sending them on their way. Our case managers help patients understand what to expect from their specialty appointment and work to remove barriers along the way. For example, a patient scheduled for a colonoscopy may not realize that preparation begins days before the procedure, so our case manager can help make sure the patient understands the instructions and is prepared.
Transportation can be another significant barrier. When appropriate, the Free Clinic may provide transportation assistance through gas cards or help patients coordinate GRTC Link transportation. Our case manager may also follow up after the appointment to make sure the patient made it to their visit and understands the next steps in their care. It's a coordinated process designed to make sure patients don't have to navigate the healthcare system alone.
Partnerships That Expand What's Possible
The Free Clinic of Powhatan cannot—and shouldn't—provide every type of medical care under one roof. Instead, we have built relationships with medical professionals and organizations that extend the reach of our clinic and give our patients access to expertise that would otherwise be difficult to obtain.
Access Now is one of those important partners. Access Now provides free specialty medical care to patients who have no health insurance and meet its eligibility requirements, including being at or below 200% of the Federal Poverty Level. For eligible Free Clinic patients, this partnership can open the door to specialty care that might otherwise be completely out of reach.
We are also fortunate to have physicians and practices who generously donate their time and expertise directly to our patients. These partnerships include:
Virginia Physicians for Women, whose providers come to the clinic monthly for our Women's Clinic, providing services such as Pap smears and breast exams.
Dr. Erin Adams, local dermatologist, who provides a monthly dermatology clinic for patients who need skin checks and specialized dermatological care.
Dr. Dave Bass and Dr. Gregg Tobey, physical therapists , which provides specialty services to our patients monthly and on an ongoing basis.
A local neurologist, who is available when our primary care providers need a specialized opinion or determine that a patient needs neurological care.
Other local physicians and practices who work with our case manager on an individual basis, sometimes providing discounted care for patients who don't qualify for Access Now.
Each partnership represents another door that can open for a patient.
When a Referral Becomes a Life-Changing Outcome
Recently, one of our new patients came to the Free Clinic with a two-year history of shoulder pain that had become significantly worse over the previous six weeks. She had already visited urgent care multiple times without finding relief, and the pain severely limited her arm mobility and was affecting her ability to work.
After evaluating her, our medical team recognized that she needed specialized orthopedic care. An urgent referral was made, and within a month, she underwent a pro bono rotator cuff surgery valued at approximately $80,000. Today, she is recovering well and participating in physical therapy through an Access Now referral. Most importantly, she is no longer experiencing the severe pain that had affected her for years.
The patient recently stopped by the clinic to share her progress and thank the team that helped connect her to care. She told us, “God opened the doors to an amazing clinic. It’s a miracle that I was able to have my surgery through a referral from the Clinic. Your clinic is a gift from God to the community.” Her story is a powerful example of what can happen when primary care, case management, community partnerships, and medical professionals come together around one patient.
The Need for Specialty Care Is Growing
The need for these connections continues to grow. In August 2025, the Free Clinic of Powhatan had facilitated 74 specialty referrals. By August 2026, that number has grown to 170—a 56% increase.
Behind every referral is a person who needed something beyond a routine primary care appointment. It might be a specialist's opinion, a diagnostic procedure, physical therapy, a women's health service, or a surgical intervention. Whatever the need, the goal is the same: help the patient get the care necessary to move forward.
Funding the First Step
When you support the Free Clinic of Powhatan, you're not simply funding a doctor's appointment. You're funding the first step that can lead to an entire continuum of care.
A patient may not know that a specialist is what they need. They may not know how to access one, have a primary care provider who can make the referral, or understand how to navigate the process once a referral is made. Even after receiving a referral, transportation, scheduling, preparation, and other barriers can stand between a patient and the care they need.
The Free Clinic helps bridge those gaps. Your support allows us to provide the primary care that identifies a need, the case management that helps patients navigate the system, and the relationships that connect our patients to specialists throughout our community. Our medical partners extend that care even further, creating a network that is far greater than what any one organization could provide alone.
