The Free Clinic of Powhatan is committed to providing quality, comprehensive health care to our low-income, underinsured residents with compassion, dignity and respect.

Eligibility Criteria

PLEASE READ CAREFULLY. To become a patient you must:

​

  • Reside in Powhatan, Amelia or Cumberland County

  • Now accepting patients in some Chesterfield County zip codes for medical only

  • Be between 19 and 64 years of age (exception: Dental patients can be 65 or older)

  • Have no insurance, (exception: Dental patients can have Medicare or Medicaid)

  • Be at or below the household income as stated in the charts shown by Clinic service;

Note:  The following are considered members of your household:  yourself, your spouse, your minor children, disabled children over the age of 18, and grandchildren for whom you have custody.  You must bring corresponding financial information for each member of your household as well.

The following required paperwork must be submitted before you can receive any services from the Clinic.

  • Proof of residency in Powhatan County or eligible surrounding county

    • Driver’s license and

    • Utility or other bill with name and current address 

  • Current 1040 Federal Income Tax form (if filed) and schedule C if you are self-employed

  • Proof of all household income for one month (as applicable):

    • Most recent month pay stub(s)

    • Unemployment award letter

    • Food stamp (SNAP) award letter

    • Social Security benefit letter 

    • Disability benefit statement letter

    • Any retirement or pension documentation

    • Child support letter

    • Medicaid Determination Letter, if applicable

    • Disability Determination Letter, if applicable

  • copy of your Medicaid card, if applicable

How to Register

To access any of the Clinic’s services, you must first  complete an application form AND BE APPROVED.  The entire process takes 2 - 3 weeks to complete.  Registration takes place in person at the times below or you can complete your paperwork online from the link listed below.  Please remember that your application must be complete AND approved prior to receiving services.

  • Mondays by appointment | 5:00 pm to 8:00 pm

  • Tuesdays by appointment  |  10:00 am to 1:00 pm

Recertification Requirements

Every twelve months from the registration date or last recertification date, all patients must recertify to remain eligible to receive services and medications from the Clinic. You must continue to meet the Clinic’s eligibility and financial criteria. The same process and financial information is required to register are also required to recertify.
